Terms of Use

These Terms of Use (“Terms”) govern your access to and use of Caregiver Readiness Passport (“we,” “us,” or “our”), including carereadypassport.com and related services (the “Service”). By creating an account, accepting an invitation, or using the Service, you agree to these Terms. If you do not agree, do not use the Service.
1. What the Service is
Caregiver Readiness Passport is an educational and caregiver-readiness platform. It helps parents or authorized adults assign Type 1 diabetes education to caregivers, track completion, and document whether assigned training has been completed.
The Service may include general education about Type 1 diabetes, Dexcom G7, Omnipod 5, finger-stick blood glucose checks, BAQSIMI, food/carbohydrate concepts, emergencies, and caregiver communication.
2. What the Service is not
The Service is not:
- A medical device.
- A healthcare provider.
- A substitute for a physician, nurse, diabetes educator, school nurse, or emergency professional.
- A substitute for an individualized written diabetes care plan, Diabetes Medical Management Plan (DMMP), emergency action plan, Section 504 plan, or other medical/school documentation.
- A source of insulin dosing, bolus calculations, correction recommendations, or personalized treatment instructions.
- A CGM monitoring service or emergency alert service.
- A guarantee that a caregiver is competent, licensed, medically certified, or safe to perform a particular procedure.
Training content provides general education. Individualized medical decisions must come from the child's current written plan, parent/guardian instructions, healthcare team, and applicable emergency instructions.
3. Emergencies
Do not rely on the Service as an emergency-response system.
If you believe a person is experiencing a medical emergency, follow the person's current emergency plan and contact 911 or the appropriate emergency service when indicated. The Service does not monitor users, glucose values, devices, or emergency conditions in real time.
4. Eligibility
Accounts are intended for adults age 18 or older. By creating or accepting an account, you represent that you meet the account-age requirement and have authority to use the Service in the role assigned to you. There are no child user accounts.
5. Account roles
Parent/Admin
A Parent/Admin may create or manage caregiver relationships, select training modules, assign courses, review progress, and use other parent features provided by the Service.
Caregiver
A Caregiver receives access through an invitation or authorized assignment and may access only the training and records made available to that caregiver.
Platform Admin
Platform Admin access is internal and is not available through public self-registration. You may not misrepresent your role or attempt to obtain privileges you have not been granted.
6. Parent/Admin responsibilities
If you use the Service as a Parent/Admin, you agree that you are responsible for:
- Having authority to assign caregiver training for the person receiving care.
- Selecting age bands, device modules, procedure modules, and other course settings accurately.
- Providing caregivers with the person's current individualized written care plan and emergency information outside or alongside the Service as appropriate.
- Keeping caregiver instructions current when medical orders, devices, treatment plans, or emergency information change.
- Deciding which caregivers should receive optional hands-on procedure training.
- Confirming any practical competency or hands-on verification that you require.
- Providing appropriate supplies and emergency contacts.
- Not relying on course completion as a substitute for clinical instructions or individualized hands-on training.
- Avoiding unnecessary entry of highly sensitive medical information into free-text fields.
7. Caregiver responsibilities
If you use the Service as a Caregiver, you agree to:
- Complete assigned training personally and honestly.
- Follow the person's current written care plan and authorized instructions.
- Ask the parent/authorized contact when you are unsure rather than improvising.
- Perform only procedures you are authorized and prepared to perform.
- Use current manufacturer instructions for device procedures when directed.
- Keep your account secure and not allow another person to complete training under your identity.
- Contact emergency services when indicated by the person's emergency plan or circumstances.
8. Training completion and practical procedures
A status such as “Training Complete,” “Readiness Passport Current,” or similar language means only that the user completed the assigned training requirements in the Service.
It does not mean that the user:
- Holds a professional license or certification.
- Has been medically credentialed.
- Has demonstrated hands-on competency unless a separate practical-verification record explicitly says so.
- Is authorized by law, a school, employer, healthcare provider, or parent to perform every procedure described in training.
Parents should independently decide which procedures a caregiver may perform and whether in-person practical verification is required.
9. Manufacturer and third-party content
The Service may reference or embed training content from Dexcom, Insulet/Omnipod, BAQSIMI's manufacturer, YouTube, Vimeo, Wistia, or other third parties.
Third-party content belongs to its respective owner and may change without notice. We do not control third-party websites, services, availability, or privacy practices. Users should follow the current official manufacturer instructions for the exact device/product being used.
References to product names or trademarks do not imply sponsorship, endorsement, or affiliation unless expressly stated.
10. Privacy
Our Privacy Policy and Consumer Health Data Privacy Notice describe how we process personal and health-related information. They are incorporated into these Terms by reference.
11. Account security
You are responsible for maintaining the security of your account and for activity performed through your credentials.
Notify us promptly at privacy@carereadypassport.com if you believe your account has been compromised.
You may not:
- Share authentication links or credentials in a way that gives another person access to your account.
- Attempt to access another user's account or data.
- Circumvent role, authorization, database security, invitation, or course restrictions.
12. Acceptable use
You may not use the Service to:
- Violate law or another person's rights.
- Attempt unauthorized access to accounts, databases, infrastructure, or source systems.
- Upload malicious code or interfere with Service availability.
- Scrape or systematically copy protected course content except as permitted by law or these Terms.
- Misrepresent course completion, practical verification, or another person's identity.
- Use the Service to make automated clinical treatment decisions.
- Enter information you do not have authority to share.
- Use the Service for harassment, discrimination, or unlawful employment/school decisions.
13. Intellectual property
Except for third-party content, the Service, software, curriculum structure, original text, graphics, design, and other materials are owned by or licensed to Caregiver Readiness Passport and are protected by applicable intellectual property laws.
We grant you a limited, personal, nonexclusive, nontransferable, revocable right to use the Service for its intended purpose.
These Terms do not transfer ownership of the Service or its content to you.
14. Feedback
If you voluntarily provide ideas, feedback, or suggestions about the Service, you permit us to use that feedback to improve the Service without compensation or obligation, provided that we do not publicly identify you without permission.
15. Availability and changes
The Service is evolving. We may add, remove, update, suspend, or discontinue features.
We may update medical/device educational content as authoritative guidance or manufacturer instructions change. Material curriculum updates may require caregivers to review updated content or complete retraining.
We do not promise uninterrupted or error-free availability.
16. Suspension and termination
We may suspend or terminate access when reasonably necessary to:
- Protect users or the Service.
- Address security threats or abuse.
- Enforce these Terms.
- Comply with law.
You may stop using the Service at any time and may request account deletion through the application's Privacy & Data settings.
17. Disclaimers
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE.”
WE DISCLAIM WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, AND ANY WARRANTY THAT THE SERVICE WILL BE ERROR-FREE, UNINTERRUPTED, OR SUITABLE FOR A PARTICULAR MEDICAL, SCHOOL, CHILDCARE, OR EMPLOYMENT SETTING.
WE DO NOT WARRANT THAT COMPLETING TRAINING WILL PREVENT A MEDICAL EVENT, ERROR, INJURY, OR EMERGENCY.
Nothing in these Terms excludes warranties or rights that cannot legally be excluded.
18. Limitation of liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, Caregiver Readiness Passport AND ITS AFFILIATES, CONTRIBUTORS, AND SERVICE PROVIDERS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, REVENUE, DATA, OR GOODWILL, ARISING FROM OR RELATED TO THE SERVICE.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, OUR AGGREGATE LIABILITY ARISING FROM OR RELATED TO THE SERVICE WILL NOT EXCEED THE GREATER OF (A) THE AMOUNT YOU PAID US FOR THE SERVICE DURING THE 12 MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM OR (B) $100.
Some jurisdictions do not permit certain limitations, so these limitations may not apply in full.
19. Indemnification
To the extent permitted by law, you agree to indemnify and hold harmless Caregiver Readiness Passport from claims, damages, liabilities, and reasonable costs arising from your unlawful use of the Service, material violation of these Terms, or unauthorized disclosure of another person's information.
This section does not require you to indemnify us for claims caused solely by our own unlawful conduct.
20. Governing law and venue
These Terms are governed by the laws of New Jersey, without regard to conflict-of-law principles, except where applicable consumer law requires otherwise.
Subject to applicable law, disputes that are not resolved informally will be brought in the state or federal courts with appropriate jurisdiction in New Jersey.
21. Changes to these Terms
We may update these Terms. If changes materially affect user rights or obligations, we will provide reasonable notice and may require users to accept the updated Terms before continuing to use the Service.
